I. Recommendation
Pursuant to Chapter XI, Section C.3.a of the Code of the Chicago Park District, the Director of Risk Management and the Director of Purchasing recommend that the Board of Commissioners of the Chicago Park District (the "Board") authorize the General Superintendent and CEO or their designee be authorized to enter into a contract with LIFESAFETY365 L.L.C., for the supply, delivery, installation, and program management of automated external defibrillators (AEDs) district-wide.

Contract Period: Three (3) year initial term with one (1) two (2) year extension option.
Contract Amount: Not-to-Exceed $1,008,040.48 for the initial three (3) year term. Expenditures remain subject to annual appropriations and each year's budget. If extension option is exercised, funding shall be based upon available annual appropriations.
Compliance Goals: The Minority and Women-Owned Business Enterprise (MBE/WBE) participation for this contract meets the compliance goals of 25% MBE participation and 5% WBE participation. The proposal for this authorization includes 95% participation with LIFESAFETY365 L.L.C as a MBE, self-preforming vendor and 5% WBE participation.
Scope of Services: Supply, delivery, installation, and program management of AEDs district-wide. Services will also include training and certifying staff across the Park District on AED usage.
