I.                     Settlement Information

Settlement Amount:                      $100,126.25

Authorization:                                          Authorize the General Counsel to execute the Settlement Agreement and include other relevant terms and conditions in the written Agreement.

 

II.                     Recommendation

 

It is recommended that an order be entered authorizing the lump sum settlement of the Workers' Compensation claim of Lisa Dolan v. Chicago Park District Case No: 17 WC 031157, subject to the approval of the Illinois Workers' Compensation Commission, in the amount of $100,126.25. The General Counsel has determined that this settlement is in the Park District's best interests.

IV.                     General Conditions

 

l) Conflicts: The agreement shall not be legally binding on the Chicago Park District if entered into in violation of the provisions of 50 ILCS 105, the Public Officer Prohibited Activities Act.

 

2)                     Ethics: The Chicago Park District's Ethics Code, Chapter III of the Code of the Chicago Park District, shall be incorporated into and made a part of the agreement.

 

3)                     Contingent Liability: The agreement shall contain a clause that any expenditure beyond the current fiscal year is deemed a contingent liability, subject to appropriation in the subsequent fiscal year budget(s).
